AD INSERTION ORDER

We require a signed Insertion Order, before we can accept your ad. Please fill the Insertion Order out completely. Also, please indicate on the Insertion Order if your ad is print ready or if we are building it, and if there is color in your ad. If you want us to design your ad, the layout, art, logos, etc. need to be here by April 15. We will bill you at the rate of $65 an hour and email a proof to you for approval. Please also read the Terms & Conditions.
